how do i repair a sliding pocket door?

do you have to tear out the wall?

No, you shouldn't have to open the wall. Pocket Door Hardware varies but they are all made to maintenance. Try removing the front hardware so that you can tilt the door into the room and slide the door past the jamb so that you can access the back hardware. Some doors just lift and remove like a sliding closet door, others are trickier but still removable, usually a screw that you loosen to disconnect the door from the hardware. The key is getting the front loose so that you can slide the door past the jamb and get to the back.
